How To Prepare For Your First Visit
Schedule your initial evaluation by submitting an appointment request form or calling our office
Bring a list of questions you may have for your provider
Bring any information regarding your medical history, medication use, x-rays/imaging results, and primary care provider information
Bring a water bottle
Prepare to arrive 10-15 minutes prior to your appointment time to complete our new patient paperwork

What to Expect at your Initial Evaluation
Our comprehensive evaluations typically take 60 minutes so we can best get to know you and your goals. We will ask you about your past medical history, any imaging performed for the problem area, and ask you about your current symptoms. Assessments and special tests may be performed including muscle testing, balance testing, range of motion assessment, palpation, and others so we can determine the best diagnosis and fully customize your treatment plan.
