How To Prepare For Your First Visit
Schedule your initial evaluation by clicking the "Book Now" button below or by navigating to our "Contact Us" page to utilize our online scheduling service
Please log in to your patient portal to complete the necessary intake form and other required documents. If you encounter any issues accessing the forms online, don't worry. We can provide you with a paper copy to complete during your visit. If this is the case, please arrive 10-15 minutes prior to your appointment to allow sufficient time for the documentation process

Bring a list of questions you may have for your provider
Bring any information regarding your medical history, medication use, x-rays/imaging results, and primary care provider information
Bring a water bottle

Our Patient Handbook is also available as a hard copy within our office for your review.
What to Expect at your Initial Evaluation
Our comprehensive evaluations typically take 60 minutes so we can best get to know you and your goals. We will ask you about your past medical history, any imaging performed for the problem area, and ask you about your current symptoms. Assessments and special tests may be performed including muscle testing, balance testing, range of motion assessment, palpation, and others so we can determine the best diagnosis and fully customize your treatment plan.
